Epidemiological and Histopathological Profile of Prostate Cancer in Conakry

Abstract: Introduction: Prostate cancer is the most common male cancer. The aim of this work was to analyze the evolution of the epidemiological and histopathological profile of prostate cancer in the urology department at the Ignace Deen National Hospital in Conakry. Materials and methods: This was a descriptive retrospective study in 10 years, from January 1, 2011 to December 31, 2020. The data was collected from records available in the archives of the Ignace National Hospital Deen of Conakry. Results: 963 cases of prostate cancer were collected, representing 75% of urogenital cancers. We have observed a constant annual growth of cases, with an annual average of 96 cases. The mean age of the patients was 67 ± 13.56 years, with extremes of [47 - 102 years]. Farmers were the most affected with 36.3% of cases; 9.9% of patients (n=92) were smokers. Patients consulted more often with a delay of more than 12 months. Prostatic adenocarcinoma was the most found histological type with 99.8% of cases. More than half of the patients (62%) had a Gleason score of 3+3 (ISUP 1). Conclusion: Prostate cancer remains frequent representing the first urological cancer in Conakry. Prostatic adenocarcinoma is the main histological type. This study encountered difficulties related to the lack of a reliable data collection system.

Abstract: Mesonephric-like adenocarcinoma of the endometrium (ML-AC) is a rare variant of endometrial adenocarcinoma with aggressive clinical behavior and unusual metastatic spread. Because of its rarity, the diagnosis may be challenging but needs to be accurate due to the mostly poor prognostic outcome of these tumors. To aid in the understanding and recognition of this entity we present the detailed histopathological, immunohistochemical and molecular analysis of a case of ML-AC clinically mimicking advanced ovarian cancer in a 55-year-old woman and put it in the context of literature research on ovarian and endometrial carcinomas. In the present case the patient revealed abdominal distension, ascites and a moderately elevated CA-125 level (504.8 U/ml) with widespread peritoneal carcinomatosis and an endometrial lesion confined to the endometrium with the presence of free-floating tumor cells within the tubal lumen, suggesting transtubal peritoneal spread of the endometrial primary. All tumor sites shared various architectural patterns on histology with negative staining for p16, WT-1 and estrogen receptor and positivity for TTF-1. Matched pair molecular analysis revealed an identic clonal KRAS-mutation (p.G12C) within the endometrial, peritoneal and lymph node involvement as well as in the recurrent tumor. There was retained mismatch repair protein staining and p53 wild type pattern without any additional mutational alterations, including POLE. A hepatic recurrence was diagnosed after 11 months. Literature study confirms our findings and the suggestion of a morphomolecular diagnostic approach additional to the fact that KRAS-mutation may represent a targetable alteration within a specialized therapeutic approach for this entity.

A Study of Symptom Experience of Hepatocellular Carcinoma Patients After Hepatic Arterial Infusion Chemotherapy Surgery

Abstract: Background: Hepatocellular carcinoma is the 2nd leading cause of cancer death among Chinese residents. Patients with cancer simultaneously experience multiple symptoms. Most of the current studies on patients with advanced hepatocellular carcinoma focus on the treatment effect, or focus on a single symptom, with less attention to the management of symptom clusters in patients. Objectives: The aim of this research is to explore the symptom experience of hepatocellular carcinoma patients after hepatic arterial infusion chemotherapy surgery and the Relationship between each symptom experience and total number of patient symptoms. Methods: A symptom Assessment Questionnaire for Patients undergoing Hepatic Arterial Infusion Chemotherapy (HAIC) was administered to a sample of 278 patients with hepatocellular carcinoma at advanced stages. Data were analyzed using descriptive statistics, Pearson correlation, and regression analyses. Results: The most striking result to emerge from the data is that the most severe symptoms in patients after HAIC surgery were Poor sleep and Decreased appetite. And presence of psychological suffering, pain, gastrointestinal discomfort, and bad mood may cause patients to develop more symptoms. Conclusions: Patients with hepatoma after HAIC surgery concurrently experienced multiple symptoms of discomfort. We should pay more attention to the symptomatic experience of these patients. A further study with more focus on the management of the most common and comorbid multiple symptoms to reduce symptoms and improve quality of life in patients with hepatocellular carcinoma is therefore suggested.

Abstract: GATA3, IKZF1, and ARID5B polymorphisms are validated risks of pediatric acute lymphoblastic leukemia (ALL) in numerous ethnicities, whilst, their role in adult patients with acute leukemia is less well defined. Objective: The study aims to investigate the association between the GATA3 rs3824662, IKZF1 (rs4132601& rs11978267), and ARID5B (rs10821936 & rs10994982) and the susceptibility to adult acute leukemia, as well as their impact on the treatment outcome. Methods: Real-time PCR was performed for the 5 SNPs in 91 patients (53 ALL and 38 AML) and 194 healthy controls. Results: IKZF1 rs4132601 G allele, G/G and G/T genotypes, as well as G/A haplotype of IKZF1 rs4132601/rs11978267 were significantly higher in ALL vs. healthy controls (p-values were <0.001, 0.001, 0.012, and 0.036, respectively) and in AML vs. healthy controls (p-values were <0.001, 0.001, 0.001, and 0.005, respectively). ARID5B rs10821936 C allele, C/C and C/T genotypes, as well as C/A haplotype of rs10821936/rs10994982 were significantly higher in ALL vs. healthy controls (p-values were <0.001, <0.001, 0.005, and 0.036, respectively), and only C allele and C/C genotype in AML vs. healthy controls (p-values were 0.002 and 0.017, respectively). Shorter Overall Survival (p = 0.003) and lack of remission (p=0.041) were significantly higher in ALL patients harboring IKZF1 rs4132601 G/G genotype. Conclusion: IKZF1 rs4132601 and ARID5B rs10821936 are potential risky SNPs for the development of acute leukemia in Egyptian adults. Furthermore, ALL patients with the IKZF1 rs4132601 GG genotype are probably more prone to poor outcome.

Abstract: Introduction: Worldwide, breast cancer is the second most common cause of death. The occurrence of breast cancer is roughly 2.3 million women of all ages worldwide. Both the normal breast cells and some breast cancer cells have the receptors that attach to the estrogen and progesterone hormone and play vital roles for normal and even abnormal growth of breast cells. The aim of this study was to evaluate the prognosis of breast cancer in relation with receptor status. Methods: This prospective observational study was carried out in the Department of Clinical Oncology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh during the period from December 2011 to December 2021. In total 100 breast cancer patients of several stages and grades were enrolled in this study as study subjects. Proper written consents were taken from all the participants before data collection. All data were processed, analyzed and disseminated by using MS Excel and SPSS version 25.0 as per need. Result: In this study, among total 100 cases, estrogen receptors were analyzed in 88 cases; among them in majority (86%), positive estrogen receptors were found whereas negative receptors were found in 14% cases. Progesterone receptors were analyzed in 85 cases; among them in majority (69%), positive progesterone receptors were found whereas negative receptors were found in 31% cases. HER 2 receptors were analyzed in 52 cases; among them in majority (67%), negative HER 2 receptors were found whereas positive receptors were found in 33% cases. highest death (12%) occurred in patients with ER-/PR-. In contrast, no death was seen in ER+/PR+ patients. In comparing the estrogen receptor status among participants, we did not find any significant correlation considering the stages or grades of carcinoma; the P values were >0.05. Conclusion: This study concluded that, compared to women of breast cancer with ER+/PR+ tumors, women with ER+/PR-, ER-/PR+, or ER-/PR- tumors experienced higher risks of mortality and distant metastasis. Receptor status of breast cancer may be considered as potential indicator for proper management of patients with breast cancer. Preliminary diagnosis and treatment of breast cancers have a certain survival advantage. Thus, we should need to assess the hormone receptor status for every patient before commencing treatment. To get robust data, multicenter studies are in great need of policymakers to interpret the demonstrable scenario and to take necessary steps towards mitigating this problem.
